Application for Service. Complete the
application and submit with your connection fee and advance payment
for two months of service.

Monthly
Service is billed three months at a time, four times a
year. The bill is received around the first of the
months of January, April, July and October. The amount
for three months of service plus tax is $38.34. The due
date for each quarter is printed on the bills. If a
payment is not received by the due date, your service
may be suspended. Service will remain suspended until
the payment and connection fee are received. Suspension
of service may occur with out notification.
